Разработка коротких, безопасных и эффективных режимов химиотерапии (ХТ) туберкулёза с лекарственной устойчивостью (ЛУ) возбудителя, на основе внедрения новых препаратов у детей и подростков, остаётся актуальной проблемой.</p></sec><sec><title>Цель</title><p>Цель: оценить эффективность и безопасность коротких режимов ХТ с включением деламанида (Dlm) у детей и подростков больных туберкулёзом с множественной, пре-широкой и широкой лекарственной устойчивостью (МЛУ/пре-ШЛУ/ШЛУ) возбудителя.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. С марта 2022 г. проводится проспективное, открытое, несравнительное исследование. Включено 37 пациентов (2–17 лет) с различными формами туберкулёза органов дыхания, которые получали Dlm и базовый режим — минимум 3 препарата. Первичный анализ эффективности: по завершении основного курса ХТ, с оценкой исходов лечения; вторичный анализ: наблюдение через 1 год — контроль рецидива туберкулёза. Оценка переносимости Dlm: кардиотоксические реакции (ЭКГ-мониторинг); нейропсихиатрические реакции (опрос, наблюдение за поведением и сном пациентов).</p></sec><sec><title>Результаты</title><p>Результаты. У 100% пациентов всех возрастных групп был зарегистрирован исход лечения — «клинически излечен». ХТ по коротким режимам проведена 83,0% — 29/35 (70,4–95.3%, 95% ДИ) пациентов. Из 29 пациентов, пролеченных по коротким режимам: 6-месячный режим проведён — 3, 9-месячный — 9, 12-месячный — 17. Наблюдение через 1 год после завершения лечения по коротким режимам проведено у 16 пациентов — рецидивов не выявлено. Взаимосвязь назначения Dlm с развитием ПР установлена в 8% (3/37) случаев — нейропсихиатрические нежелательные реакции.</p></sec><sec><title>Заключение</title><p>Заключение. Применение Dlm в сочетании с другими препаратами в коротких режимах ХТ туберкулёза с МЛУ/пре-ШЛУ/ШЛУ МБТ при проведении комплексного лечения демонстрирует высокий процент достижения успешного результата.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The development of short, safe, and effective chemotherapy regimens for drug-resistant tuberculosis in children and adolescents — based on the introduction of new drugs — remains a pressing issue. The aim of the study was to evaluate the effectiveness and safety of short-course chemotherapy regimens incorporating delamanid (Dlm) in children and adolescents with tuberculosis caused by multidrug-resistant, pre-extensively drug-resistant, or extensively drug-resistant (MDR/pre-XDR/XDR) strains.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. A prospective, open-label, non-comparative study has been conducted since March 2022. The study enrolled 37 patients (aged 2–17 years) with various forms of respiratory tuberculosis who received Dlm in combination with a standard regimen consisting of at least three drugs. The primary efficacy analysis was performed upon completion of the main course of chemotherapy, involving an assessment of treatment outcomes; the secondary analysis consisted of a follow-up visit at one year to monitor for tuberculosis recurrence. The assessment of Dlm tolerability was based on: cardiotoxic reactions (ECG monitoring); neuropsychiatric reactions (patient interviews, observation of behaviour and sleep).</p></sec><sec><title>Results</title><p>Results. The treatment outcome recorded was «clinically cured» for 100% of patients across all age groups. Short-course chemotherapy regimens were administered to 83.0% (29/35) of patients (70.4–95.3%, 95% CI). Of the 29 patients treated with short-course regimens: 3 received a 6-month regimen, 9 received a 9-month regimen, and 17 received a 12-month regimen. Follow-up was conducted one year after the completion of shortcourse treatment regimens in 16 patients; no relapses were detected. An association between the administration of Dlm and the development of adverse reactions was established in 8% (3/37) of cases — specifically, neuropsychiatric adverse reactions.</p></sec><sec><title>Conclusion</title><p>Conclusion.
